After each workshop, students are asked to answer five simple questions on their experience by completing the Institution’s impact assessment form. The form provides us with information on how to improve and develop our education outreach. There is a printable version of the form you can download and use at sessions if needed. The information captured will need to be input into the form below. All the information provided is confidential and used exclusively for education purposes. The form will take approximately two minutes to complete.
